Your patient is set to discharge at the end of the week, but you know their needs are high and their level of support at home won't be. How do you help ensure they're going to be prepared to go home and not get lost in the shuffle? How can SLPs best care for our patients after they're no longer under our care? Alicia is passionate about considering the patient's big picture, making recommendations that help the patient with more than what's on their plate, and adequately preparing them for the next stage of their recovery and return to their typical lives. She is a champion for interdisciplinary care and partnering with our case management and social work colleagues to ensure no patients are left to figure it all out on their own!
