Don't just set goals and forget about the goals, you have to remind yourself of your goals and your why's.
If you remind yourself every day of what you're trying to accomplish and why you're doing it. You will be more likely to do it. It's like a compound effect.
Just write your top 3 goals down, every single day. It doesn't matter how big these goals are or how impossible they are.
If you write them down often enough, your brain will believe it's possible.
