So he could insert his head through it and then by slightly pulling on it, making the belt buckle fall and close the loop tight around his neck.

As he was sliding forward and down, the fan was slowly tilting down under the strain. And by this extending the reach of the belt and thus giving him some time to make his last words. But the fan was secured very solid and after tilting down a little it stopped doing it. With his head ending up now only couple of inches away from the floor. And with his body weight at that angle starting to strangle him significantly more.

Why he just could not roll off the bed? Because he did not have a LEVERAGE to swing his body and legs. In order for him to roll his feet off the bed, he would need to push his neck inside that noose against it. Since it was the only point of contact on which to push. But that would mean an even more tight strangulation, which would have resulted in closing neck arteries immediately and him passing out right away.

If the fan tilted down just a tiny bit more or the length of the belt or scarf was just a few inches longer, then after his head would have touched the floor, he could have used his forehead against the floor as the leverage point and could have tried to swing his body and legs sideways, from pillows to the floor. But those less than couple of inches short and his disregard for risks involved in doing solo strangulation, had led to his death.
