Use the concept of “positivity” in introducing ways to bring about improvement in your leaders. You always want to emphasize the positive using the open and close and sandwich the “suggestion for improvement” in the middle. 

You can also develop a "matrix" of sorts for each leadership position in your department or organization. This will make future hiring and interviews much easier in comparing candidates for the position.
