Heparin – ACS – Notarzt

Hallo Freunde der klinischen Notfallmedizin und wieder geht es weiter und diesmal mit einem neuen Format wenn man so will, denn ich spreche nicht über einen Algorithmus sonder über ein Medikament bei einer bestimmten Patientenpräsentation.

Außerdem gibt es ein paar allgemeine Gedanken zum Selbstverständnis des Notarztes im Rettungsdienst.

Fall:

Patienty ~75 Jahre, Heimbewohnerin, Rüstig
Präklinisch mit Schock und eingeschränktem GCS aufgefunden
Blutdruck 70/30, GCS 5
EKG Senkungen ubiquitär besonders II, aVF, V3-V6; keine Hebungen, kein Block, keine T-Neg
Nach BD Steigerung durch Push-Dose (NorEpi 1mg in 250ml) GCS 9-10
Patenty gibt Schmerzen und Luftnot an.
Therapie:
Push-Dose NorEpi
ASS 250mg i.v.
Heparin 5000 U i.v.
O2 per Maske
Fahrstrecke bis in ZINA ca. 5‘, Transport im Heim unproblematisch  Gesamt ~10‘
