The inquest revealed Swale had not received formal diabetes training in over 30 years, failed to notify VicRoads for more than 20 years after his 1994 diagnosis, and never informed his insurance company. Despite his blood glucose level dropping to 2.9 before the crash and his monitor alarm sounding 10 times, Swale continued driving and has since apologised to victims' families.
