This podcast, excerpted from a September 2019 conversation event, in collaboration with the Independent Care System (ICS), provided a platform for questions to be answered on how health plans and providers can improve health outcomes for this population by addressing gaps in care coordination that often disrupt continuity of care and lead to fragmentation of care. This conversation focused on the key elements of and strategies to improve care coordination, including the role of interdisciplinary teams.
