The sequence where Elisabeth gets ready for her first injection is really fantastic. Moore's character must work out how to use the different packets and medical items in the kit on her own because the substance's manufacturers have not provided many instructions. The change is genuinely creative as Elisabeth, naked in a bathroom with white tiles, feels as though her spinal column is opening to allow her to give birth and her eye is turning double in its socket. When Elisabeth's twin appears and looks in the mirror, Qualley is the one staring back at her rather than Moore. The persona played by Qualley, who goes by Sue at first, immediately goes on the audition circuit to take Elisabeth's position on the aerobics show. There is not much new in "The Substance."
