What we get here is a Sliding Doors situation, in which Donna is forced to relive a key moment in her life (though it didn't feel like a key moment to her at the time) and make a different decision. This leads to a different version of her life unfolding, in which she never met the Doctor...but the new version of her life leads to a new version of the whole world's course of existence, and it's pretty ugly for most of us.
