Elevate, a pilot program for healthy aging in Perham, Minnesota has the mission to "curate opportunities for adults to improve their physical, mental, and social health so they can live happier, healthier lives." Elevate is one of four pilot programs funded by Leading Age Minnesota Foundation and UCare Minnesota to address the expected increase of healthcare concerns for a growing population of adults.
