Apply online

Fill out the form to begin the process. Include your contact information as well as a summary of your medical health condition. Once completed, click the submit now' button to submit the form.

Get evaluated online

A licenced mental health professional will then review the form. When discussing your mental health and the need for an ESA, be open and honest with the LMHP.

Receive an ESA letter

If the LMHP determines that your mental health condition qualifies, you will receive an ESA letter via email. You can either download or print your ESA letter.
