Unmet goals are a direct reflection of our rehab outcomes. When a discharge is planned, all goals should be met. Ask the following 3 questions when discharge planning to avoid unmet goals and negative outcomes: 1. Which goals are unmet? Focus on these areas ONLY prior to discharge 2. Does the goal need to be downgraded or discharged? If the patient will not achieve the established goal by the planned discharge date, the goal is NOT appropriate 3. Is there a patient/caregiver education and training goal? This ensures a safe and properly trained transition helping avoid RTA.
