The Equifax hack captured the data from an estimated 143 Million people (myself included) and as of September 26th, it claimed the jobs of the CIO, CISO and the CEO of Equifax.  

It's unfathomable that the breach could have been enabled by the username password combination 'admin/admin' but it appears that may have been the case.  Regardless, we talk about the event as well as ways you can mitigate the risk should it happen to you.
