1. Story (theme) Dr. G recently presented to NISP about the late stages of life: the last 2-10 years of life depending on illness burden. How do we prevent CRISIS MODE and create smoother transitions of care? EDUCATION of referral sources (ex. hospitals, case managers, fiduciary associations https://pfac-pro.org/ ) and the public about optimal discharge plans to maintain a sense of PURPOSE in the lives of seniors.
2. Educate (Teaching point) What is the CONTINUUM OF CARE? It is beyond the end of life, consider PRIOR to that: Aging in place at home, or move to RCFEs, home care, mobile physicians, transportation, home health for physical therapy (after a fall and hospitalization), palliative care, hospice. Seniors want to stay at home. Unique models like Parentis Health can catch the time BEFORE seniors go into the hospital, by providing a FULL spectrum of care from very beginning, for stability. This continuum of care or “Umbrella” of services gives seniors 55 & older purpose IN THEIR HOME, prevents crises, mitigates caregiver burnout.
