There is a great opportunity to improve care and reduce readmissions with properly executed care coordination. To understand these opportunities and also acknowledge some of the challenges that care transitions present we sat down with Dr. Tony Linares, Medical Director and Regional Vice President at Anthem National Accounts, Anthem Blue Cross, and Hank Mayers, President of Reliatech Consulting, at HIMSS15. Dr. Linares and Hank also serve as the Chair and Co-Chair of the HIMSS Care Coordination Task Force and are able to provide some insights to the future of care transitions.
