Care transitions have evolved over the past decade and Tom Ferry, Co-Founder and CEO of Curaspan, has anticipated this evolution. After a personal experience over fifteen years ago, Tom started Curaspan when he recognized that the care transition process needed new clinical workflows and technology to support better patient care.
