The sixth podcast in this series emphasizes documentation improvement for providers. In this program, Joe Nichols, MD, provides an overview of documentation requirements for the ICD-10 code set and the required changes to improve existing clinical documentation.

Dr. Nichols, previously in orthopedic practice, has been involved full-time in health IT for the past 15 years. He is now the principal for Health Data Consulting and a certified ICD-10 coding trainer. Earlier in his career, he was director of healthcare solutions for an electronic data interchange software company in Seattle, Wash.
