According to the Centers for Disease Control and Prevention (CDC), six in 10 American adults have one chronic disease and four in 10 have two or more chronic conditions. Risk adjustment aims to identify and treat these highest risk patients.

Strong patient stratification and care management programs are the best way to close gaps in care, reduce costs, and ultimately achieve better health outcomes. But getting started with risk adjustment isn't always easy — doing it right is even harder.

Beth Israel Lahey Health Performance Network (BILH-PN) began a small pilot program to see if they could make an impact. They took a creative approach to risk in their network by proactively reaching out to get patients in the office and using refined documentation processes to track progress. Their small pilot program is now expanding.
