GPH 20: Priority Setting, Social Values, and Public Health

Public health is always conducted under constraint.

No country - regardless of wealth - can fund every beneficial intervention, treat every condition at maximal intensity, or eliminate all risks simultaneously. The discipline of priority setting emerges from this reality.

This chapter explores the frameworks used to determine which health problems receive attention, funding, and policy support. It examines how burden of disease metrics, cost-effectiveness analysis, and economic evaluation intersect with ethical principles such as equity, fairness, and social justice.

We consider the tension between:

* Maximising total population health

* Reducing inequalities

* Protecting vulnerable groups

* Respecting autonomy and rights

Should resources go to interventions that save the greatest number of lives?Or to those that protect the most disadvantaged?Or to those that deliver the greatest long-term societal benefit?

Priority setting is not merely technical. It is moral and political.

The chapter also explores formal decision-making tools such as DALYs and QALYs, incremental cost-effectiveness ratios, and programme budgeting and marginal analysis - while recognising their limitations. Numbers guide decisions; they do not replace values.

Ultimately, priority setting reflects what a society believes health is for - and whose health counts.

Public health planning is therefore an exercise in applied ethics as much as epidemiology.
