GPH 31: Case–Control Studies

Case–control studies reverse the usual direction of inquiry.

Instead of following exposed individuals forward in time, we begin with those who already have the outcome (cases) and compare them to those who do not (controls). We then look backward to assess prior exposure.

This chapter explores the design, strengths, and limitations of case–control methodology.

Case–control studies are particularly powerful for:

* Investigating rare diseases

* Studying diseases with long latency periods

* Rapid outbreak investigation

* Situations where cohort follow-up is impractical

The chapter details key design considerations:

* Selection of appropriate controls

* Avoidance of selection bias

* Minimising recall bias

* Matching and its implications

* Calculation and interpretation of the odds ratio

Because incidence cannot be directly measured, the odds ratio becomes the principal measure of association - approximating relative risk when disease is rare.

The chapter emphasises methodological rigour. Poor control selection can invalidate findings. Measurement bias can distort associations.

When properly conducted, case–control studies are efficient, powerful, and foundational to causal discovery.

They allow epidemiology to reconstruct exposure histories - turning present disease into a window into the past.
