GPH 41: Systematic Reviews and Meta-analysis

Listen Later

In this episode, Medlock Holmes turns from individual studies to the architecture of accumulated evidence. One study may suggest. Several may hint. But how do we know when the totality of research points decisively in one direction?

We explore the principles of systematic reviews - structured, transparent, reproducible approaches to identifying, appraising, and synthesising all relevant evidence on a specific question. Holmes walks us through the disciplined methodology: predefined protocols, comprehensive search strategies, inclusion and exclusion criteria, risk-of-bias assessment, and structured reporting.

We then step into the statistical dimension: meta-analysis. How do we quantitatively combine results across studies? What are fixed-effect and random-effects models? How do we assess heterogeneity, publication bias, and small-study effects?

The episode also confronts the limitations of synthesis. Poor-quality studies do not become high-quality evidence when combined. Garbage in still produces garbage out. Holmes examines forest plots, funnel plots, and the logic of weighting studies by precision.

Finally, we consider how systematic reviews inform clinical guidelines, policy decisions, and global recommendations - and why transparency and reproducibility are essential in an era of information overload.

This episode reveals that evidence-based public health is not about individual brilliance - it is about disciplined aggregation.

Key Takeaways

* Systematic reviews use predefined methods to minimise bias in evidence synthesis.

* Meta-analysis statistically combines results to produce pooled estimates.

* Heterogeneity must be examined before interpreting pooled effects.

* Publication bias can distort apparent conclusions.

* Study quality assessment is essential to meaningful synthesis.

* Transparent protocols strengthen credibility and reproducibility.

* Policy decisions often rely on systematic review evidence.
