It was a mislabeled container of radioactive materials, sent by Los Alamos Labs to the WIPP site in Carlsbad, whose danger was covered up in Los Alamos, before the barrel of waste blew up underground, exposed a dozen workers to radiation and forced the WIPP site to close for 3 ½ years and counting.  Patrick Malone of the Center for Public Integrity says both the errors and the dishonesty are parts of a malign pattern of mismanagement inside the national nuclear weapons industry.  Firing the Los Alamos management is supposed to change that…but why did it take 3 years before the fired managers left their jobs?
