Issues in Projects - how can we handle, better still, prevent them!

However much we plan a project issues come up. Project issues are disruptive & puts everyone in crisis mode.  Why do issues come up? 

Ideally they should not occur when there is a Project Risks management strategy in place. As we know issues are known after they happen catching us by surprise. Risks on the other hand are mostly (maybe upto 85 to 90%) known in advance & we are prepared for them if & when they occur. 

Issues are probably those unknown unknowns which can’t be identified in advance. Admitting that issues will come up what can the project leader do to handle them so that they can be resolved as quickly & efficiently as possible ?  

An Issue Log or Register is a necessity to begin with.   This Issue Log should be maintained on a board for all to see & regularly updated.  Issues can be controlled/prevented by identifying & preparing responses to handle impediments to daily activities in a to do list 

Assumptions, constraints & risks if handled well can also prevent Issues from coming up.
