Upon completion of this activity, participants should be better able to: Recognize the impact of social determinants of health (SDOH) on cardiovascular disease outcomes, focusing on nonvalvular atrial fibrillation (NVAF); Describe patient-centered approaches to enhance the detection and assessment of adverse SDOH in patients with NVAF; and Outline community-based strategies to optimize care for patients with NVAF experiencing disparities.
