Recently hospital leaders in case management and post-acute care from throughout the Greater Los Angeles area gathered for the National Readmission Prevention Collaborative’s Los Angeles ACO, Bundled Payment, and Readmissions Summit and served on a panel that was moderated by PopHealth Podcast host Gavin Ward.   The panelists took a moment to share their thoughts with PopHealth Podcast.  Organizations represented included UCLA Health, Cedars Sinai Medical Center, Providence Health and Service, Kaiser Permanente, White Memorial Medical Center, and Fountain Valley Regional Medical Center
