Every discipline from scientists, entrepreneurs, and even artists use trial and error to move their work forward.  But as we grow up, we begin to perceive that we have more to lose from “failure” than we have to gain.  The result is that innovation stagnates and we don’t learn from our failures because we are too intent on covering them up.  Individuals and organizations that recognize this head on are able to outlearn and outgrow their competition.
