Japan Airlines Flight 123 crashed in 1985 after a faulty bulkhead repair caused a mid-air explosion that destroyed the aircraft’s hydraulics, killing 520 people.
In 1989, a freight train in San Bernardino derailed due to incorrect weight calculations and undisclosed braking failures, crashing into a residential neighborhood. The wreckage also damaged a pipeline, causing a massive fuel explosion two weeks later.
Both disasters show how minor human errors and maintenance flaws can trigger deadly chains of events with catastrophic consequences.
