Connection shares the newest grant awarded to the nonprofit by MN Dept of Human

Services, Age-Friendly MN Community Grants for the Community Health Alliance

work in Anoka and Hennepin Counties. We discuss how we plan to build on the

roadmap we developed in St Louis Park during the pandemic to scale this model

to other communities to serve our aging populations. This is a collaborative

space that will have additional episodes that we will share with our listeners

with more community risk reduction updates.
