Medical personnel are professionals. Many times the line is crossed and too much familiarity results in a medical setting.  Patients have boundaries that are expected NOT to be crossed. Abuses are happening much more frequently and to the detriment of our society as a whole.

Niacin depletion in its most severe form is called Pellagra. The brain loses function. 
Pellagra is a serious nutritional deficiency disease caused by a lack of niacin (vitamin B3)

 or tryptophan in the diet, often characterized by the "4 Ds": dermatitis, diarrhea, dementia, and death if untreated. It causes a symmetrical, sun-sensitive rash, cognitive decline, and digestive issues. Treatment involves increasing niacin intake. 

This niacin treatment has been going on for nearly 100 years. In the 1960s, Bill Wilson, one of the founders of AA, was a major proponent of using large doses of Niacin (Vitamin B3) to treat alcoholism and depression. The AA Board did not agree because the treatment was controversial.
