Rachael Slack warned the authorities about her son's father, Andrew Cairns.

He had threatened to kill her and they knew that.

Like Tania Moore five years previously, Derbyshire Police failed to act on these warnings.

As a result, an entirely avoidable tragedy occured. A tragedy that left a community shocked and a family scared by grief.
