Perfect Care is the brainchild of Kevin W. Lobdell MD, System Director at Sanger Heart & Vascular Institute and Clinical Professor-Wake Forest University School of Medicine (which are a part of Advocate Health, headquartered in Charlotte NC). Shannon Crotwell, RN, BSN, CCRN has led the administrative and clinical implementation efforts of Perfect Care since its inception in 2018.

Perfect Care aims to elevate the engagement of patients and their family, reduce barriers to care, & leverage rare, talented clinicians with technology. With the visionary and generous support of The Duke Endowment and leadership of Sanger Heart & Vascular Institute + Advocate Health, we have enrolled > 2250 patients in Perfect Care.

Of note, our results with the first 1000 consecutive Perfect Care heart surgery patients are world-class: ZERO mortality. ~17% shorter postoperative length of stay (1 day), 33% fewer 30-day readmissions (4.7%), and 97.8% discharged to home.
