The Michigan Primary Care Association (MPCA) recently partnered with the AMA to implement the AMA MAP™ Hypertension Quality Improvement Program at two pilot health centers. After this collaboration resulted in a 6 percent improvement in hypertension control, it’s now expanding to all other MPCA health centers and could even focus on other conditions like diabetes. Grace Health in Michigan recently implemented the AMA MAP™ Hypertension Quality Improvement Program, and now, pharmacists play a much bigger role in helping both providers and patients. The AMA MAP™ Hypertension program is designed to enable physicians and care teams to achieve rapid and sustained improvements in blood pressure control among their patients with hypertension. In the fall of 2020, Cook County Health, which is one of the largest public health systems in the country based in Chicago, collaborated with the AMA to implement this program, and here to share the results with Dr. Jennifer Caudle are Dr. Chantal Tinfang and Ms. Kesha Love. Dr. Tinfang is an attending physician of family medicine at Provident Hospital at Cook County Sengstacke Primary Care Clinic, and Ms. Love is a registered nurse and Director of Health Information Technology Interoperability Services at Cook County Health.
