When a team appears to have an issue, Dr. Graham often advises managers to observe their team's interactions outside the office to identify underlying issues. As a fellow training and development professional myself, I used to tell managers to take their teams bowling to observe the team dynamics and then we would talk about what interventions might need to happen to strengthen team dynamics.
