In the Marine Corps we received Pro and Con marks as young marines and as we advanced in rank these became a more detailed fitness report.  They cover every thing from physical fitness to military education, accomplishments, awards, disciplinary action and an overall evaluation of your job performance.  

In most circumstances these reports are accompanied by a formal counseling so you can actually hold a conversation about your strengths and weaknesses.  
So it only seems prudent to counsel yourself on our life goals and your efforts toward reaching them.  

Why would you counsel yourself?
 To gain perspective on your efforts
 To gain impartial insight into a situation and 
your response
 To identify behaviors and habits that are contrary to your goals
 To evaluate and adjust your goals and expectations

How often should you counsel yourself?
On regular intervals
When needed to establish new goals
As needed for perspective
On the spot to correct a behavior

Where do you start?
  With your current situation
 With the current event
 With your needs and goals
 With your behavior and reactions

Topics for counseling 
(effort)
 Effort put into work
 Intent put behind the effort
 Steps toward goals
 Your resources and efforts toward building them (financial, material, emotional, supporting)
 Goal timelines (realistic or not)

(conduct)
 Emotional triggers
 Fears and anticipations
 Emotional maturity
 Reactions to stimulating events
 Overall attitude

(proficiency)
  Strengths
 Weaknesses
 Obstacles
 Self Improvement
 Mentors and mentees

How do you keep track of the key points?
 Journaling
 Taking note of setbacks
 Audio recordings 
 Immediate analyzation
 Wet board or chalk board

At the end of the day take stock in what you have accomplished toward your goals. 
In the morning plan one to three tasks that move the needle.

Develop a routine that gets you going in the morning.

Celebrate the victories and show gratitude
Reward yourself for moving forward.

Take joy in your accomplishments even if they are not as fruitful as you had hoped. 

Be honest, lying to yourself is the only game you can play where there is no winner but you always lose.
