Inner-city populations remain at high risk for poor health outcomes, and premature death due to preventable diseases and environmental factors associated with known socioeconomic and primary health care inequities. In many of these communities COVID-19 spreads at faster rates than other communities. We will take a look at inner city communities in and around Baltimore and ask the questions: Where are we in the midst of COVID 19 and what is the outlook for tomorrow? How do we positively impact health outcomes at the individual, family and community level looking at population health management, standards of care, and selfcare. Profound Conversations is a moderated discussion with a question and answer segment for listeners to contribute thoughts and seek guidance.
