On May 22nd, 2026, FDNY responded to a shipyard on Staten Island for reports of smoke and two missing workers. What started as a smoke investigation turned into a confined space rescue operation — and then an explosion that injured 36 people, killed one civilian, and seriously injured two members of FDNY.
The cause is still under investigation. But the confined space protocols that could have prevented this have existed for decades.
