The Community Health Alliance began in St Louis Park when the fire department and Care Resource Connection partnered to solve the historical problem of connecting our most vulnerable to needed resources, thus reducing the dependency on the 911 system. Now after nearly 8 years the program has evolved into an effective deliverer of services managed under a structured care coordination system designed by Care Resource Connection. Firefighters, as the most trusted community resource connect with patients with a simple question “Are you open and willing to accept help”. Invariably the answer is yes, and from that point the program begins to work for them. These firefighters who serve at the tip of the spear share their observations of the value they see in the lives of our patients and for fire departments considering their options, how it positively impacts those who want to make a difference.
