Many patients with pulmonary hypertension live outside of the normal patient referral channels or do not have ready access to PH centers of excellence. Further, their primary care provider, if they have one, may not be aware of or comfortable with referral processes to PH centers of excellence. This roundtable of experts explores some of the issues surrounding gathering in these potential "lost" patients. Potential solutions to gathering these hard-to-reach patients under the umbrella of PH care are considered.
