We set expectations and hire below expectations and wonder why we are a revolving door and under performing.

Set the expectation and raise everyone to it. If you provide the tools, training and leadership and they don't meet it then its time to cut it. 

As a leader it is all your fault. Its your fault if they fail, succeed or keep them too long when they aren't a good fit.
