On this episode of The Sonography Lounge hosts Lori and Trisha sit down with Dr. Charlotte Derr & Dr. Allyson Hansen to discuss the GUARD Protocol (Gastric Ultrasound for Aspiration Risk Determination).
With the increased use of GLP-1 Agonists (Semaglutide, with brand names such as Ozempic, Wegovy, Rybelsus, Etc) for both diabetes and weight loss, there has been an increased number of aspirations in patients undergoing procedures. One of the effects of GLP-1 agonists is delayed gastric emptying which keeps patients feeling "full" thus reducing the desire to eat and overall caloric intake and blood sugar variations. Typically 6-8 hours not eating is enough time for a patient about to undergo an elective procedure to digest and empty their stomachs thus reducing the risk of aspiration while under anesthesia. With patients on GLP-1 agonists, we are seeing gastric contents remaining for several days after the last meal which can increase the risk of aspiration.
Dr. Derr and Dr. Hansen, discuss how they are using ultrasound and the GUARD protocol to determine the volume of food in a patient's stomach before elective and emergency procedures. This quick and simple bedside technique can drastically alter a provider's pre-procedure course of action.
CHARLOTTE DERR, MD, RDMS, FPD-AEMUS, FACEP
Dr. Derr is the Professor of Emergency Medicine and the Fellowship Director of the Advanced Emergency Medicine Ultrasound Fellowship Program at the University of South Florida Morsani College of Medicine in Tampa, FL and serves as the Co-Medical Director at Gulfcoast Ultrasound Institute.
She is a fellow of the American College of Emergency Physicians and is a registered diagnostic medical sonographer. She has also obtained the Focused Practice Designation in Advanced Emergency Medicine US from the ABEM.
She has authored and co-authored numerous articles, book chapters, and other publications on ultrasound and lectures throughout the US and internationally on diagnostic ultrasound.
ALLYSON HANSEN, DO
Dr. Allyson Hansen is the Ultrasound Director at the University of South Florida Emergency Medicine program. She received her education at the University of South Florida and Campbell University. Dr. Hansen completed her Hackensack University Medical Center residency and Emergency Ultrasound Fellowship at Tampa General Hospital. She is passionate about teaching and is an instructor at Gulfcoast Ultrasound Institute.

JAMES MATEER, MD, RDMS
Dr. James Mateer was one of the pioneers in Emergency Medicine Ultrasound applications and was among the first Emergency Physicians to obtain their registry for Diagnostic Medical Sonography from the ARDMS.
Dr. Mateer has served as the Medical Director of Gulfcoast Ultrasound Institute since 1996. He is co-editor for the textbook: “Emergency Ultrasound” and has authored numerous chapters and articles on Emergency Medicine and Emergency Ultrasound applications. He has lectured extensively throughout the United States and Internationally.

David P. Bahner, MD FAAEM, FACEP, FAIUM
Dr. Bahner is Professor and the Ultrasound Division Director Department of Emergency Medicine at The Ohio State University Medical Center, Columbus, OH. He has specialty training in hyperbaric medicine, emergency medicine research and an advanced teaching fellowship with ACEP. He was Membership Chair, AIUM and served on the Board of Directors, at the Society of Ultrasound in Medical Education. He is currently the AIUM delegate to the AMA and an ex oficio member of the AIUM Board of Governors.
He has earned fellow status as faculty of AAEM, ACEP and AIUM. He has multiple publications, written book chapters, letters and inventions as an advocate for point of care ultrasound. Dr. Bahner has published more than 60 articles on point of care ultrasound and education. He has lectured internationally and nationally on this subject, and continues to champion clinician performed ultrasound for better patient care and patient safety as the Ultrasound Division Director within the OSU Department of Emergency Medicine.
Andrew Laudenbach, MD
Dr. Laudenbach is the Assistant Director of Emergency Ultrasound and Co-Director of the Emergency Ultrasound Fellowship program at Hennepin Healthcare in Minneapolis. He is an assistant Professor of Emergency Medicine at the University of Minnesota Medical School.

Dr. John Broussard, (https://www.thealliancerx.com/) is board certified in Family Medicine and Sports Medicine and specializes in both regenerative and osteopathic manipulative medicine. He earned his Doctor of Osteopathic Medicine degree from Nova Southeastern University in South Florida and completed his residency training in Family Medicine at an osteopathic hospital, practicing in the urgent care setting. He then followed to complete a Sports Medicine Fellowship at the Institute for Non-Surgical Orthopedics in Florida. In 2014, he integrated ultrasound into his practice for diagnostics and guided interventional procedures. In 2020, he and his brother opened their own practice right here in sunny St. Pete. Together, they help active adults and athletes quickly overcome injury and get back to the activities they love.
Dr. David Wang, (https://www.rosm.org/) Who earned his Doctor of Osteopathic Medicine degree from The West Virginia School of Osteopathic Medicine, where he was selected as a teaching Fellow in Osteopathic Manipulative Medicine. He completed his residency training in Physical Medicine and Rehabilitation at Harvard Medical School/Spaulding Rehabilitation Hospital, during which time he was appointed Chief Resident. He is a founding partner and Director of Education and Training of Regenerative Orthopedics and Sports Medicine, with offices throughout the Metropolitan DC area. He specializes in diagnostic musculoskeletal ultrasound, non-fluoroscopic interventional spine, osteopathic manual treatment, and regenerative medicine including prolotherapy, PRP, orthobiologics, and lipoaspirate stem cell therapies.
Dr. Imran Siddiqui, (https://www.rosm.org/) Currently serves as a Partner and the Director of Clinical Operations at Regenerative Orthopedics and Sports Medicine in Washington, DC. He graduated medical school Cum Laude from Thomas Jefferson University where he was inducted in to the Alpha Omega Alpha honor society. After which, he completed his residency at Harvard Medical School and Spaulding Rehabilitation Hospital where he trained extensively in musculoskeletal ultrasound and regenerative medicine and was awarded resident teacher of the year. Currently, Dr. Siddiqui also servers as an ultrasound and instructor and board member for the Interventional Orthobiologic Foundation. He also has instructed at Harvard Medical School, Orthosound Educational Institute, and the American Academy of PM&R.
