The Vincennes Shootdown and Express Samina Sinking

In 1988, Iran Air Flight 655 was mistakenly shot down by the USS Vincennes, which misidentified the civilian airliner as a fighter jet, killing all 290 people on board.
In 2000, the Greek ferry Express Samina struck a well-marked rock formation due to crew negligence and mechanical failures, resulting in 80 deaths.
Both tragedies show that while technology played a role, human error, poor decision-making, and panic were the primary causes, turning routine transit into catastrophic loss of life.
