The elderly mobster Frank Costello (Robert De Niro) gets shot in the head and attacked in 1957 while traveling to his residence in New York City. Frank miraculously recovers from the attack and recognizes right away the work of Vito Genovese (De Niro), his old buddy and criminal world partner, who has recently invested in the heroin and opium trade and is trying to seize control of the company. Before prohibition threatened their empire and sent Vito abroad while Frank kept up the underground, Frank and Vito had a lengthy history together.
