Whether seeking to blame individuals, governments or nations, it seems obvious that there's a time when it's appropriate to look for root cause, to establish what went wrong and a time for focusing instead on solving the problem at hand. In this episode I explore the idea of when it's the right time to look for fault and cause, and when we'd be better off getting on and trying to solve the problem at hand.
