Annie Tulkin explains why she started Accessible College, who she serves, and when families & students should begin to engage with her. Contrary to what I (Eric) thought, you don't want to wait until your senior year of high school - there is a LOT of benefit to starting as early as your freshman year.
Planning to attend college when you have health conditions and/or physical disabilities requires a lot more than just thinking about what accommodations you should ask your college's disabilities services office for - you should also be thinking about the surrounding area and what is available. For example, if you have appointments with specialists every 6 months will you be able to continue these while you're in school? Will the local grocery stores have the foods you eat? Etc.
You can learn more about Annie on her website or any of her social media accounts.
