Serious illness care is provided by a team of specialists, including physicians, nurse practitioners, and social workers who work together with a patient’s other doctors to provide an extra layer of support.
It is appropriate at any age and any stage in a serious illness, and it can be provided in conjunction with curative treatment.

The learning platform addresses real issues, with in-depth/digestible lessons, practical steps, and guided support from a care manager.

Frances Hall is the co-founder and executive director of ACAPcommunity, a non-profit that provides a variety of educational programs that offer information, resources, support and community for adult-children as they care for their aging parents and for themselves.
