    Reflex Sympathetic Dystrophy – RSD – Re-examined
Hector Rivera is a coach, filmmaker, and podcaster who suffers from Reflex Sympathetic Dystrophy. His website and podcast; Happy and Healthy Over 40 are committed to people over 40 who want to maintain a healthy mind in a powerful body. He’s been involved in the entertainment business for over 20 years.
He started his journey as an executive coach and men’s work facilitator, due to a heavy piece of equipment that fell on top of him while working. The healing process went poorly. Hector was ultimately diagnosed with Reflex Sympathetic Dystrophy (RSD), a rare disorder of the sympathetic nervous system that is characterized by chronic, severe pain.
Hector was determined to get better. He followed the instructions of his doctors to the letter. He underwent multiple surgeries and saw many specialists such as orthopedic surgeons, podiatrists, and neurologists. Nothing worked. In fact, Hector was told at the outset, that medical science had no cure for this type of neuropathy. All the doctors could do was to try and reduce his level of pain.

  • 237 Subdural Hematoma Resolved Without Brain Surgery – Knopman
Brain Surgery Takes A Remarkable New Turn With Subdural Hematoma
Jared Knopman, MD is a board-certified neurosurgeon and interventional neuroradiologist who specializes in cerebrovascular disorders, including aneurysms, AVM’s, brain tumors, and carotid occlusive disease, as well as subdural hematomas.
This interview is another cutting-edge report from Weill-Cornell Brain and Spine, following the previous exceedingly interesting report using Ultrasound for Essential Tremor with Dr. Kaplitt here at CBJ/232.
Minimally Invasive – Without Opening The Skull
Dr. Knopman reports here on his considerable experience in the use of embolization of aneurysms and Arterial-Venous Malformations [AVM’s] to prevent further bleeding and death. He is also deeply experienced in the use of carotid stenting/endarterectomy, and intra-arterial chemotherapy.
In addition, he performs spinal cord stimulation and microvascular decompression for chronic pain syndromes and trigeminal neuralgia. He is one of the few neurosurgeons in New York City with dual expertise in both open neurosurgical as well as minimally invasive interventional techniques for the treatment of neurologic disease, which affords him a unique perspective in determining the optimal treatment for each individual patient’s needs using the most cutting-edge technologies.
Dr. Knopman has been named to the SuperDoctors Rising Stars list, a distinction afforded the top 2.5% of physicians in his field. His expertise is widely sought and he serves as a consultant to other neurosurgeons, traveling the country to assist them in their development of advanced stenting techniques with complex aneurysms.
Just What Is This New Vascular Procedure?
Until recently, most subdural hematomas that needed to be removed required open brain surgery, with its own risks and complications, but now there is a new minimally invasive approach to these dangerous vascular roadblocks, pioneered at Weill Cornell Medicine Brain and Spine Center.  Known as an MMA1 embolization, this new procedure is performed by a surgeon with expertise in image-guided cerebrovascular treatments, who threads a microcatheter from a tiny needle into the upper thigh, up to the arteries feeding the hematoma, to cut off its blood supply.
Dr. Jared Knopman is the first and only neurosurgeon performing this procedure anywhere, and he is currently engaged in a clinical trial that is also yielding important new information about the nature of these clots.
    Signs and Symptoms
Subdural hematoma symptoms typically include a headache, vomiting, drowsiness, dizziness, confusion, differing pupil size, slurred speech and eventual loss of consciousness. Symptoms may appear immediately, but in some cases, they can take weeks or even months to manifest, as was the case with Chicago White Sox pitcher Danny Farquhar, who collapsed in the dugout during a game April 20, was rushed to the hospital, diagnosed with an aneurysm and immediately operated on. (As of April 23, he was still in critical condition.)
    Who Is Most Susceptible?
Elderly people are at highest risk for chronic subdural hematoma because brain shrinkage causes these tiny veins to be more stretched and more vulnerable to tearing. Seniors are also more susceptible to falling as they get older, and more likely to be on blood-thinning medications, which can contribute to hemorrhaging in case of trauma. Infants are also susceptible to falls, accidents or abuse that can cause a hematoma to form.
    It can also occur spontaneously,

  • 236 Mushrooms Inform & Heal The Mind – Chilton
    Mushrooms, Mind & Healing
The parts you could see – what most people called a mushroom – was just a brief apparition. A cloud flower.      ~ Margaret Atwood
Jeff Chilton, raised in Pacific Northwest, studied ethnomycology at the University of Washington in the late sixties. In 1973 he started work on a commercial mushroom farm in Olympia, Washington. During the next 10 years, he became the production manager, responsible for the cultivation of over 2 million pounds of Agaricus mushrooms per year. He was also involved in the research and development of shiitake, oyster and enoki mushrooms which resulted in the earliest US fresh shiitake sales in 1978.
    Street Experience
In the 1980’s he operated a mushroom spawn business and in 1989 he started Nammex, a company that introduced medicinal mushrooms to the US nutritional supplement industry. He traveled extensively in China during the 1990’s, attending conferences and visiting research facilities and mushroom farms. In 1997, he organized the first workshop for organic certification of mushrooms in China.
    Considering Mushroom Science
A founding member of the World Society for Mushroom Biology and Mushroom Products in 1994 and a Member of the International Society for Mushroom Science, Mr. Chilton’s company was the first to offer a complete line of Certified Organic mushroom extracts to the US nutritional supplement industry. Nammex extracts are used by many supplement companies and are noted for their high quality based on scientific analysis of the active compounds.

    Jeff’s Mission
He challenges the industry for higher standards and transparency with mushroom products. He wants people to have access to quality products and have the knowledge to properly identify these products in the marketplace.

Dissociative Identity Disorder – Fusion Through ‘As If’
Unexpressed emotions will never die. They are buried alive and will come forth later in uglier ways.
    ~ Signund Freud
Dr. Earl W. Flora provides therapy in Williamsburg, VA, for all ages with a wide range of individual mental health issues, family, work, and related issues. His specialty is treating Dissociative Identity Disorder, and here he adds this interesting perspective to reintegration/fusion.
Along with a full range of psychological issues, Dr. Flora utilizes a well-publicized but seldom used single sentence to quickly assess & identify DID clients. In addition, he has developed an abbreviated technique for working with clients suffering from Dissociative Identity Disorder by avoiding the use of abreaction to bring the personas together, without forcing them to relive the negative effects of the event that caused their separation in the first place.
It’s about time… time to recover.
Only after spending 27 years developing and validating the technique has he published his book titled Then They Were One – Fusion Without Abreaction,  in which he describes his technique in great detail.

  • 233 The Pain Companion – Recovery Insights – Shockley
The Pain Companion for Chronic Pain: Recovery Insights
Sarah Anne Shockley is the author of The Pain Companion. In the Fall of 2007, she contracted Thoracic Outlet Syndrome (TOS), which is a collapse of the area between the clavicles and first ribs and has lived with debilitating nerve pain ever since.
Why She Wrote The Pain Companion
“As you probably already know, being injured or ill to the point of debilitation is an incredibly intense experience. Besides feeling a certain level of shock from the trauma of having your whole world turned upside down, there are a multitude of other emotional and psychological reactions that arise. These include loneliness, confusion, loss, frustration, depression, and hopelessness.
Also, a person undergoes an incredible amount of stress when unable to function adequately in their world. There are financial worries about how to pay for the necessities of life if you can no longer work. There are worries about how to physically cope with the everyday tasks most of us used to take for granted, such as being able to care for children and family, cook, clean, shop, drive, take out the garbage, and even handle personal hygiene.
Any or all of these activities may have become physically impossible or extraordinarily laborious and painful.  Also, being in constant pain is utterly exhausting, and there are minimal mental, emotional, or physical resources left over to participate in life anything like a reasonable person. Social interactions may become few and far between. With all this going on, it can become a huge struggle to get up in the morning and carry on.”
Photo by Stefano Pollio on Unsplash
    Sarah’s Mission
“In the midst of all this, I was hoping to find some help somewhere for these multiple challenges. What I discovered was that most of the books on pain address physical approaches to pain management, and most are written by practitioners. While they are undoubtedly valuable for many, my condition does not do well with physical exercises, so they weren’t useful to me, and I was seeking a certain level of solace and understanding.
I wanted to hear from someone like me. I wanted to hear from someone who understood the daily challenges of actually living in pain, with pain, and having all of your experiences stem from or are colored by pain.”
    Healing Answers
“Since there was little available to me regarding treatments, either because I couldn’t afford them or because they didn’t exist, I was pretty much left to my own devices. I found that continually trying to battle with pain – get rid of it, avoid it, not feel it, or push it away – was more exhausting and frustrating than it was productive. What I needed was a healing modality which included pain as part of my healing journey as an integral part of the process rather than putting so much energy into eradicating it.
Over time, I created my meditative exercises to understand the role of continued pain in my body and eventually began working with pain as an ally in healing. I found ways to live with pain and not sink into a mire of depression or self-pity. With these approaches, the acute level of pain I was in began to shift. Eventually, I decided that writing about it would not only clarify this approach for my use; it would be helpful for others who live with chronic pain. Out of these experiences came The Pain Companion.”

  • 232 Essential Tremor – High Intensity Focused Ultrasound – Kaplitt
Essential Tremor Treated With High-Intensity Focused Ultrasound
Dr. Michael Kaplitt is the first doctor in New York to use high-intensity focused ultrasound [HIFU] to relieve a patient’s Essential Tremor – ET. The procedure is part of a new clinical trial testing the use of this technology to eliminate the source of tremors in a completely non-invasive way. Dr. Kaplitt and his neurosurgical team, along with Dr. Levi Chazen from Radiology and Dr. Harini Sarva from Neurology, were able to watch as the patient’s tremor diminished visibly during the procedure. [Listen to his patient Steve here at CBJ as he tells you what it  felt like coming out of HIFU treatment.]
Essential tremor may be mild in some individuals, but in others, it can become debilitating over time. The tremors, which are often most severe in the hands, can interfere with everyday activities such as eating, drinking, writing or tying shoelaces.
Patient Experience with Dr. Kaplitt’s Team
We asked for this interview after working with For a previous CBJ report from a healed golfer-patient treated by Dr. Kaplitt with HIFU listen here: CBJ/221.  This Ultrasound tool and the Tremor Treatment Team at Weill Cornell Brain & Spine is simply amazing.
Current treatment options for ET include medication as well as Deep-Brain Stimulation (DBS), which involves implanting a battery-operated neurotransmitter under the collarbone connected to a wire that runs up the length of the neck into the scalp, where it is guided to the brain through a small hole in the skull. The tip of this wire sends the electrical impulses generated by the neurotransmitter into the precise spot in the brain that regulates the activity of the key circuits in essential tremor. In a phrase, brain surgery.
Michael Kaplitt MD, Ph.D., and the Weill Cornell team are also national leaders in the use of DBS for essential tremor, Parkinson’s disease, epilepsy and a variety of other disorders for more than 15 years.
Listen up as Dr. Kaplitt discusses with us this remarkable new, non-invasive procedure to correct ET without brain surgery. – And he answers some of my questions about Parkinson’s as well.

    Additional Research Work
Dr. Kaplitt has pioneered human gene therapy for neurodegenerative disorders, having performed as a student the first rodent study that used the adeno-associated virus (AAV) vehicle for gene transfer to the brain.
He performed the world’s first human gene therapy procedure for Parkinson’s disease and more recently helped to guide the first successful randomized, double-blind, sham-controlled trial of this experimental treatment, which was the first positive trial of this kind for gene therapy in any brain disorder.
    On The Psychiatric Side
He is also pioneering the development of novel deep brain stimulation treatments for psychiatric conditions, including refractory drug addiction. Dr. Kaplitt is an internationally recognized expert in the science of gene therapy and molecular neurobiology, and his current focus is to understand molecular disease mechanisms and develop novel molecular therapies for Parkinson’s disease, depression, addiction, and pain.
He has published over 100 articles and has edited two books on these ...

  • 231 Weight Loss & Emotional Eating – Simon
Weight Loss: The Neurobiology of Appetite, Eating, and Weight Gain
Julie M. Simon explains that when we eat in the absence of physical hunger cues, routinely choose unhealthy comfort foods, or eat beyond full, something is out of balance. Weight loss becomes impossible.
She says that these tendencies suggest that we are missing important self-care skills generally learned in childhood. Current brain science shows that a lack of consistent emotional nurturance in infancy and childhood, when the brain is being formed, can result in difficulties with self-regulation. In her work here at CBJ Julie uses her extensive personal and client-based experience and offers a comprehensive, step-by-step mindfulness program designed to rewire the brain and end overindulging once and for all.
In this interview, we discuss food and food dysregulation, compromised executive function challenges regarding food intake, with direct insights into an improved recovery process – including and beyond dieting.
