Dr. Jesse Jacobs has taken on the mission to create "The Gold Standard of Chiropractic" through his company, FreeForm Chiropractic. He opened his first office in 2016, and grew it to become a $2M practice and a "lighthouse" in his community.

With 6 offices and growing, Dr. Jacobs has instilled his mission into every FreeForm office – TO HELP PEOPLE LIVE FREE. Live free of subluxations, medications, and healthcare dependency. Dr. Jacobs believes that when you pursue your full vitalistic health potential and achieve it, you become unstoppable.

Dr. Jacobs is married to his wife Emily, with 4 children- Kai, Koa, Keanu, and Kenna. He lives his life through the FOUR CORE VALUES - Faith, Family, Fortitude and Freedom. He runs a mentorship mastermind called The Vitalis Movement, which helps chiropractors & chiropractic students achieve Freedom in their life through these core four values.
