  • Functional Medicine 1.0 Is Broken. Here’s What Replaces It.

Most clinicians stepping into functional medicine don't fail because they lack passion or training.
They fail because they drift into what Dr. Z calls the $20,000 mistake zone, and it's a lot easier to get there than most people realize.
Functional Medicine 1.0 trained practitioners to worship the guru on stage explaining an enzyme three steps deep into a hormone cascade. Nobody taught them how to translate that to patient care at 9am Monday morning. Nobody taught them how to run a practice. And nobody taught them how to get patients to actually do the work, which turns out to be the real medicine.
Functional Medicine 2.0 fixes all of it. Three rules. Real demos. A new model for 2026.
    Learn more about:
Functional Medicine 1.0 is broken — not because the clinical knowledge is wrong, but because it trained memorization over decision-making, data over transformation, and lone-wolf practitioners over CEO clinicians with systems and teams.
The $20,000 mistake zone is real — and fear of expensive mistakes combined with overwhelm is exactly what keeps practitioners stuck before they ever start.
Root cause detective work is the easy part. Getting patients to change is the real medicine. Functional Medicine 2.0 is built around that distinction.
You cannot scale a practice if you are the entire staff. Functional Medicine 2.0 is about team, systems, health coaches, and you operating as the CEO clinician — not just the technician.
This episode is a beginner's blueprint for 2026 — whether you're starting, reinventing, or opening an additional practice, with tool demos to show what the push-button version actually looks like.
If you’re quietly thinking, “Yep… this old model is breaking me,” you’re not alone.
FM2 is where clinicians go when they’re done pretending the current system is fine.

  • Language Is Your Compliance Shield.

The words you choose in health and wellness are not a style preference. They are your compliance shield.

There is a difference between "you are anemic" and "there are some flags here, take these labs to your primary care physician to check for anemia." One is a diagnosis. One is health and wellness education. And if you're working without a medical license, or choosing not to practice under the one you have, that distinction is the line between a business that's protected and one that isn't.

In this episode, Dr. Z walks through the language rules, business setup considerations, and practical tools every health and wellness practitioner needs to stay compliant and build confidently.

    In this episode:

    • Language is compliance. "You have inflammation" is a diagnosis. "This supports a healthy inflammation response" is education. One protects you. One doesn't.
    • Use supplement brand public-facing catalogs as a language baseline. Their attorneys already vetted it. Don't go further than they go.
    • Business setup matters — entity type, liability insurance, client agreements, intake language, proper disclaimers. Health and wellness liability can run as low as $250 a year. Medical malpractice is a very different number.
    • FTC and FDA marketing rules apply regardless of your license. Social media freedom is not claim freedom. Stay in your lane online.
    • The legal tool inside the Dr. Z Way maps your specific license, your state, what you can and can't do, two business model options, and the exact questions to bring to your attorney and CPA. You go in informed or you get jerked around. Those are the options.

If you’re quietly thinking, “Yep… this old model is breaking me,” you’re not alone.

  • Your Credential Sets Your Scope. It Doesn't Set Your Practice.

You've been told what you can't do.

Can't have a practice without the right license. Can't touch labs. Can't do referral programs. Can't build a real business.

    Most of that is wrong.

In this episode, Dr. Z breaks down two legal paths for health and wellness practitioners — including one that almost nobody knows about — and exactly what's available to you depending on your license, your goals, and how you want to structure your business.

    In this episode:

    • Path A is health and wellness coaching — cash-based, you set your rates, works across state lines in most cases, legal referral and affiliate programs, access to labs and supplements.
    • Labs are not off limits. Having access to labs through a certification doesn't mean you can diagnose or treat medically.
    • Supplement margins matter more than most people realize. Up to 65% on practitioner-grade supplements versus the 30-35% most practitioners start with on standard platforms.
    • Path B — the MSO model — is for practitioners who want a true medical practice beyond their license scope.
    • The biggest mistake practitioners make is putting one foot on both sides — practicing under a medical license while also trying to do things outside it.

If you’re quietly thinking, “Yep… this old model is breaking me,” you’re not alone.

  • I Stopped My Treatment for a Year. Here's What Happened.

Dr. Z knew why she needed treatment. She still wanted a break from it.

An insurance interruption left her without regular immunoglobulin therapy for her primary immunodeficiency. Then came a viral illness, a severe health setback and a daily routine she hated—but needed.

In this solo episode of Functional Medicine 2.0, Dr. Z shares what it was like to manage post-viral symptoms, orthostatic hypotension, mast cell activation symptoms and new food reactions while trying to keep living and running a business.

For functional medicine practitioners, the question is uncomfortable: how much do we understand about what we’re asking patients to carry out?

    Inside this episode:

    • The difference between knowing treatment matters and wanting to keep doing it.
    • How an insurance interruption and a viral illness featured in Dr. Z’s setback.
    • What managing meals, medications and basic daily activities demanded of her.
    • Why symptom relief and investigating the underlying problem both mattered in her care.

  • I Got Trolled for This Video. It Made $1M Anyway.

Dr. Z got trolled for a marketing video with frizzy hair and a blurry Zoom call. It made over $1,000,000, and she still hasn't watched it back.

In this episode of Functional Medicine 2.0, Dr. Z breaks down why she films every video in one take. She explains why waiting to "feel ready" is the most expensive marketing mistake a practitioner can make. And she makes the case that your practice's story matters more than the production value behind it.

    This episode answers:

    • Why did Dr. Z's most trolled marketing video become her most profitable?

    • Why does one take beat 45 takes for a 30-second video?

    • Do you need a big production budget to market a functional medicine practice?

    • Why does waiting to "feel ready" quietly kill practitioner marketing?

  • Your Staff Isn't the Problem. You Are.

If you're the practitioner telling everyone that your staff are idiots and nobody can do anything as well as you can — that's not an observation. That's a confession.

It means you haven't built the system.

In this solo episode, Dr. Z gets into why high achievers in private practice are the most likely to fall into this trap, what it actually costs you, and how to stop micromanaging long enough to build something that can run without your finger in every little thing.

    Learn all about:

    • Saying nobody can do the job as well as you can is lazy. It's not an assessment of your staff — it's evidence that you haven't built a system they can actually work.
    • Systems first, people second. Always. When something goes wrong, look at the system before you look at the person.
    • There are four phases to handing off any task — understanding and creation, deployment and learning, ownership, and ongoing maintenance with quality control.
    • If you take something back and do it yourself because someone didn't do it the way you wanted after one try — that's also lazy. You gave them a task and expected magic.
    • On the other side of micromanaging is space. Space to be creative, generate new ideas, and do the things that only you can do. That's what systems buy you.

  • You're Marketing Like A Mattress Sale. That's Why They Won't Book.

Most people on social media are doom scrolling at 11pm, hiding in the bathroom with their phone, or watching Finding Nemo for the thousandth time while secretly scrolling.

They don't know you. They don't care about you. And you're hitting them with "Book a call now — half off!"

    That's a mattress sale. That's not marketing.

In this episode, Dr. Z breaks down friction — what it is, where it lives in your practice, and why auditing every step of your funnel through the eyes of someone who has never heard of you is one of the most important things you can do for your business right now.

    Learn all about:

    • Most people are on social media for escapism, not because they're ready to book a consult.

    • The more steps you add, the more people you lose. You have to look at it through someone else's eyes.

    • Audit your marketing, your sign-up process, and your fulfillment. Where do people drop off in your video? Where do they stop reading? Where does the EMR confuse them? Where does the portal lose them? Find it. Fix it.

    • You're not building for the one percent who will succeed no matter what. You're building for the tech-challenged mom juggling four kids, the person working night shifts, the person who's never done anything like this before.

    • Less friction in marketing means more leads. Less friction in sales means more signups. Less friction in fulfillment means better patient results. It compounds in every direction.

  • You Have the Certifications. Where Are the Patients?

Thousands of dollars in certifications. Still no patients.

    Should you get another one?

No. And this answer is coming from someone who loves certifications, creates them, and sells them.

In this Q&A episode, Dr. Z answers three questions straight from practitioners — why certifications alone won't grow your practice, whether it's ever okay to fire a patient, and how to know when you've ordered enough labs versus when you're just ordering out of fear.

    In this episode:

    • Certifications live on the fulfillment side of the triangle. Marketing and sales are the other two sides. If you don't have people coming in the door, it doesn't matter how brilliant you are at solving the medical mystery. There's nobody there to help.

    • You're not going to just focus on marketing. You're going to master marketing, sales, and fulfillment. You're the CEO of your practice. You have to know enough to direct it even when other people are doing it.

    • Yes, it is okay to fire a patient. Dr. Z has done it more than once. The reason was always the same — mistreating clinical staff. It might feel like money walking out the door. It's actually the most important message you can send your team: I will back you up.

    • Labs: only order what's going to change the care plan. If it's not going to make a difference to clinical decision-making, it doesn't go on the order. Involve the patient in the discussion, prioritize the list, and explain your reasoning. A ranked list of five tests where they choose three is better than surprising them with a $2,000 bill.

    • LabDx just won an award — the only tool of its kind with over 75 labs, all the markers, clinical significance, therapeutic interventions, even supplement recommendations by name and dosage. And it tells you which lab to order first for that specific patient.

  • Is Your Personality Turning Patients Away?

You had a great consult. You covered everything. You felt confident.

    And they didn't sign up.

It might not be your clinical knowledge. It might not be your price. It might be that you walked into a consult with a cheerleader patient as a full-on scholar practitioner — and you lost them somewhere around methylation and enzymatic function while they were quietly checking out.

In this episode, Dr. Z breaks down all six character codes and exactly what each personality type needs from a no-charge consult — and what will send them straight out the door.

    You’ll learn about:

    • The class president patient wants structure, a clear plan, and your certainty above everything else. Hum and haw for even a moment and they're done. Their turn-off is vague advice, casual tone, anything that reads as not taking it seriously.

    • The cheerleader patient wants warmth, encouragement, something new and exciting. Add a social or group component and you've got them. Overwhelm them with data or deliver it cold and dry and they're gone before the call ends.

    • The actor patient wants your full attention, a little drama, some emotion. They want the kind of story where you had to go to Herculean effort to help them. Data dumps and dry clinical delivery? They've left the building.

    • The activist patient needs connection, presence, to feel truly heard. If you walked in distracted, moving fast, barking orders — they're not signing up. Hard sales tactics and ego are instant turn-offs.

    • The artist patient wants uniqueness and a personalized approach. Tell them you do it the same way with everyone and watch them disappear. The scholar is the only one who actually wants every detail — and they're going to need time, data, maybe a spreadsheet, before they decide.

    • The faster decision-makers are class president, cheerleader, and actor. The slower ones are activist, artist, and scholar. Know which one is sitting across from you — and adjust.

  • No, the FDA Did NOT Just Approve Peptides

Everyone's excited. The FDA is meeting and they're going to approve peptides.

    Except that's not what's happening.

The organization that gives recommendations to the FDA is meeting. The FDA has already stated they are not promoting or on the side of approving these compounds. And Alabama just became the first state to formally prohibit prescribing, administering, and dispensing research grade peptides — with a pointed reminder that research grade patient waivers offer zero legal protection.

This episode isn't really about peptides. It's about what happens when science, patient demand, and social media outpace regulation — and what you're going to do in your practice when that gap keeps widening.

    Learn all about:

    • Alabama's Board of Medical Examiners issued a formal joint notice in May 2026 making the state the first to formally prohibit research-grade peptides in practice. The warning that research-grade patient waivers offer no legal protection is the part every prescriber needs to sit with.

    • Peptides currently live in a gray zone — not a prescription, not a supplement. Access is not the same as authority. Authority is not the same as legal protection. If you're a prescriber using peptides in practice, an IRB is currently the only approved route. Know your state.

    • Dr. Z uses peptides personally. She also doesn't do things in her practice unless she has full legal authority to do it. Those two things can both be true at the same time. You don't have to be the test case.

    • The bigger issue isn't peptides specifically. It's what happens when you have the license, the training, the experience, and a patient demanding care that regulation hasn't caught up with yet. Where do you land on that?

    • The story of the physician who was medically gaslit in the ER after his own colonoscopy complication — dismissed because his pulse didn't hit the standard trigger, even though as an athlete his elevated pulse was meaningful. The nurses saved his life. The experience cost him his career and his practice.
