Host Edward di Girolamo speaks with world renowned HBOT expert, Dr. Paul G. Harch on treating Traumatic Brain Injuries and PTSD with hyperbaric oxygen therapy.
di Girolamo starts the conversation telling a story about him, Sergeant Major Simon Lemay and retired Navy Captain Jim Hooker visiting the Durham VA, near Duke Hospital, in North Carolina. They had a sit-down meeting with three heads of the Psychiatry group at Veteran Affairs. They asked the question, “Does traumatic brain injury involve any sort of inflammation?” and the response they received was, “Next thing you’re going to tell me is that hyperbaric oxygen is good for every indication. And that’s when I glaze over and say I’m not interested in talking about it.” di Girolamo shakes his head at the negligence; and vocalizes his frustration that this powerful, natural anti-inflammatory treatment exists in hyperbaric oxygen therapy, and the VA won’t even consider learning more about the benefit of using it on veterans with a traumatic brain injury.
Harch agrees that the VA is completely oblivious to the evidence that HBOT works to treat inflammation and that inflammation absolutely exists as a result of PTSD and TBI. He explains that hyperbaric oxygen is highly effective for neurological applications, and it’s most effective in white matter diseases such as brain decompression illness, traumatic brain injury, and even in cases like multiple sclerosis and carbon monoxide poisoning.
Veteran suicides are a growing number that we must get under control. di Girolamo gives praise that at least the North Carolina Legislature acknowledged HBOT as an effective treatment for PTSD and TBI. Under Senate Bill 442 they allocated $150,000 to help veterans get 40 or more Hyperbaric Oxygen Therapy treatments. These veterans are improving like never before, lives are being saved and stories are being told. They are finally getting their lives back, thanks to the power of Hyperbaric Oxygen Therapy!
But, how will HBOT help the average person – sport athlete, car accident victim, etc – with a concussion or TBI? Dr. Harch refers to the brain like a head of broccoli sitting on its stem. If it’s shaken on the stem, then the torque forces cause the flimsy white matter tracks to be damaged and many of them can then divide, resulting in the formation of wounds. Essentially a bruise is formed on the brain…. and inflammation is present, even years later. But, when we expose that area to oxygen and pressure you now get gene stimulation, inhibition of inflammation, and stimulation of repair and growth of tissue. You start growing new axons, or nerve fibers, which is the cable transmission of neurons in the brain.
Dr. Harch stresses that if you or someone you know has recently had a brain injury, please find a hyperbaric facility as soon as possible. If done early enough, the brain does not need too many HBOT treatments to repair itself; but if you wait years, when you’re still having headaches and other post-concussive syndrome symptoms, it could take 40 or more HBOT treatments to reverse the damage. Dr. Harch exclaims with confidence that HBOT is a “Slam Dunk” for TBI and PTSD recovery and 99% of the time it will absolutely help every patient who comes through his doors.
In this final episode of HBOT News Conversations with Dr. Harch, host Edward di Girolamo takes this time to review a handful of indications we have yet to discuss.

Dr. Harch sums up his time at HBOT News Network by reminding everyone how Hyperbaric Oxygen Therapy started, in relationship to treating many ill patients at once. It was during the Spanish Flu pandemic, which is essentially a relative of the coronavirus pandemic, when Dr. Cunningham created a large hyperbaric chamber from spare parts of an old boiler. People started flocking to him for treatment because it was literally saving the lives of Spanish Flu victims who were on their last breath. The pressure alone was enough to save the lives of many, but imagine if they had antibiotics then to treat the secondary pneumonia that came from the viral infection — so many more lives could have been saved. Dr. Harch recommends Vance Trimble’s book, “The Uncertain Miracle” to learn more about the use of Hyperbaric Oxygen Therapy during that time period. He stresses that today the medical industry isn’t reinventing the wheel with HBOT, they’ve just studied it longer, figured it out better, and witnessed the miraculous effects of healing with HBOT for a large variety of indications.
di Girolamo ends the segment asking Dr. Harch, “If God could give you one miracle related to Hyperbaric Oxygen that you could request and get. What would that miracle be?” Dr. Harch responds that he wishes for the mass distribution and access of Hyperbaric Oxygen Therapy. He wishes for the day that HBOT could be readily accessed and available to anyone with no barriers to receive it.

di Girolamo starts this conversation with Dr. Harch bringing up the 8,101 genes that are activated by Hyperbaric Oxygen Therapy. The discussion leads to talk of Big Pharma, telomeres, reparative gene process, patents, and successful business models for HBOT clinics.
Dr. Harch explains that Hyperbaric Oxygen Therapy affects 8,101 of our genes, and it’s doing so on a different level for the ones that are overactive and abnormal in the disease process. He speaks of a 2004 lecture he gave to A4M about the effects of HBOT on longevity; and asks, “Where is senescence and aging based?”, answering with, “It’s in our DNA”. Dr. Harch dives in to how HBOT turns on our genes to grow new tissue and stimulates cellular repair and regeneration. di Girolamo agrees and brings up a different past lecture from Dr. Harch where Harch referred to Hyperbaric Oxygen Therapy as the ‘Fountain of Youth‘.
This series ends with Dr. Harch discussing a Buffalo VA study, and how the study subjects (comprised of older veterans) literally came alive and started propositioning nurses after Hyperbaric Oxygen Therapy treatments. He explained this is why no follow-up study was ever done because they weren’t expecting such and didn’t know what to do. Makes sense…. studies have shown success in HBOT treating erectile dysfunction.

Host Edward di Girolamo speaks with world renowned HBOT expert, Dr. Paul G. Harch about Autism.
The percentage of children with Autism is increasing at an alarming rate. In order to successfully treat Autism and navigate around the roadblocks, Dr. Harch explains that it’s important to understand that Autism is not a psychiatric diagnosis. Autism is a wounding condition of the brain, without all of the potential contributors being identified to the wounding process.
Dr. Harch was the first to treat a series of children with Autism using Hyperbaric Oxygen Therapy; a therapy which in and of itself is neuroplastic (it is a neuroplast — affecting neuroplasticity). He states that there’s a plethora of information on the fact that Autism is a physical brain abnormality, and that’s what’s causing this Autism phenomenon. Functional imaging scans that have been done on these Autistic children shows 90% of them have a problem involving primarily the temporal lobes and frontal lobes; mostly temporal lobes. Dr. Harch has treated a good series of these children with Hyperbaric Oxygen Therapy and about 80% of them will respond to the treatment. There are various causes, which may explain why 100% don’t respond to HBOT, since there are different insults to the brains and/or it’s environmental. But, Dr. Harch stresses that there’s no such thing as a genetic epidemic, so this Autism epidemic we’re seeing must either be caused by environmental factors or infectious agents.
Dr. Harch affirms that the underlying nature of Autism is that there’s a dominant immune dysfunction in these children that often manifests in the GI tract, but also in the brain. Dr. Harch recommends our viewers read this beautiful article by Dr. Dan Rossignol, a Family Practice Physician with two autistic sons. In the article Dr. Rossignol reviews all of the pathophysiology — the disease processes — that have been identified in Autistic children, and he reviews all of the science of Hyperbaric Oxygen Therapy and the effect it has on all of those disease processes. In the end, HBOT is shown to be a good match for the treatment of Autism.
The studies have been controversial and complicated on Hyperbaric Oxygen Therapy for Autism. But contrary to the results of these studies, Dr. Harch is here to say that the proof is in the imaging. He has imaging revealing that after only one treatment — sometimes it’s a series of treatments — changes are seen in these children’s brains, and there’s improvement in profusion (such as in the temporal lobes).
Dr. Harch professes that what he’s been trying to do all these years, these decades, is to answer a simple question, “Does Hyperbaric Oxygen Therapy work for your condition or not?” He explains it’s critical to start with HBOT solely and not throw a bunch of other therapies into the mix because then you have too many variables, and you won’t know exactly what’s helping and what’s not. He explains Hyperbaric Oxygen Therapy is a foundation biological therapy, and declares there is no identified therapy out there that can do what Hyperbaric Oxygen can do; including activating 40% of our genome.
Dr. Harch references Chapter 12 of his book, The Oxygen Revolution, and says “I’m sorry but if your doctor is giving you negative information, flatly ignore him! Go get this therapy for your child!” Treating with HBOT is no different than if you dosed the child with a drug, or a drug combination. But what exactly happens during a stroke and how can HBOT help?
We have more blood flow happening in the parts of the brain that are working hard to coordinate the movement of muscles, the formation of words, etc. Dr. Harch refers to this blood flow like the RPMs in a motor, the blood flow revs up and increases as we increase our movement & activity. However, having a stroke causes a blockage or lowering of blood flow to the brain, which can cause “electrical silence” – when the cells get to a point at about 60% reduction in blood flow where they no longer have enough oxygen (and blood flow, sugar, and fat) to generate electrical signals. These cells do not die, they live in a state of idling.
Dr. Harch explains there was a strong misunderstanding among the medical community that brain cells were either alive or dead after a stroke or injury. Over the years, HBOT has proven to the medical community that brain cells can just be in an idle/injured state and under the right conditions (like oxygen + pressure) they can be revived to function again.

In Dr. Harch’s book he describes the trip down the birth canal as one of the most high-risk journeys any human will ever make. Dr. Harch educates us on how traumatic birth experiences can trigger Cerebral Palsy diagnoses in young infants, and how premature babies are at great risk for Cerebral Palsy since they are born with the center of the brain undeveloped, still very fragile and prone to spontaneous bleeding.

Additionally, Dr. Proefrock’s presentation from the same conference, “Current Trends in Naturopathic Treatment of Pediatric Neurological Injury” reveals stem cell data for children in the midst of HBOT treatment; showing that the children who have had Hyperbaric treatments produced 4x the amount of stem cells compared to those who did not.
Dr. Harch explains the roadblock for using Hyperbaric Oxygen for treatment of CP is still extremely difficult to get around, and he references the 2012 article by Novak and Badawai, Last breath: effectiveness of hyperbaric oxygen treatment for Cerebral Palsy as an example. In this article, they concluded that “Hyperbaric Oxygen does not have a clinically important effect on gross motor and self-care function in children with Cerebral Palsy.” Dr. Harch clarifies why they were wrong on the science, and how information like that is what’s preventing the average medical doctor to recommend HBOT for Cerebral Palsy.

di Girolamo asks Dr. Harch to explain his background, and the road he had to take to get to where he is today.
In Episode 4, Dr. Harch tells a miraculous story of how his medical journey with Hyperbaric Oxygen Therapy fell into his lap during his surgical residency. It was during this internship when Dr, Harch was in a horrendous vehicle accident and nearly killed. This led to more than one medical leave of absence. While out on his second leave of absence after yet another surgery, his perspective changed; and he started to wonder what he wanted to do with his life.
After not finding fulfilment in an emergency medicine practice, Dr. Harch decided that he needed to follow his true passion and interest if he was going to have any chance of feeling accomplished, and changing lives.
Dr. Harch had a high level of interest in Navy divers and the extreme cases of brain decompression sickness. He started asking questions about how they were being treated for decompression sickness and why they were not getting better. Dr, Harch refers to the unbelievable cases that landed in his lap as his “Manna from Heaven”. Other doctors tossed these cases to the side, but Dr. Harch decided to try something different with these patients – he treated using lower doses of hyperbaric oxygen therapy. It totally reversed what was going on with these patients!
Even though his results were clinically astonishing, the Medical Societies were surprisingly not impressed or welcoming; forcing Dr. Harch to create more research documentation and do more clinical work to prove his success with hyperbaric oxygen therapy to treat chronic brain issues. During his journey to prove the results, he was implemental in successfully being able to show brain decompression illness on imaging. The imaging results were so dramatic after hyperbaric oxygen therapy that the radiologists didn’t even know how to read it! This is about the time when Dr. Harch’s road to patient care took a straight-shot towards healing a larger variety of indications with HBOT.
His goal today is the same as it was back then…. prove to everyone that Hyperbaric Oxygen Therapy WORKS! Dr. Harch has no plans to retire and stresses that he wants to see this all over the finish line.
Dr. Harch has used HBOT to change the lives of children and adults; but it was his own life that was also greatly changed by the power of hyperbarics. HBOT News podcast host, Edward di Girolamo, talks with special guest and HBOT expert, Dr. Paul G. Harch, MD. This is the first of nine episodes with Dr. Harch examining the God-given miracle of Hyperbaric Oxygen Therapy.
In this episode, we dive into how HBOT is a game-changer for inflammation. Harch explains that inflammation is the driving factor for countless chronic illnesses, but thankfully Hyperbaric Oxygen Therapy is a natural proven way to suppress inflammation.
Harch summarizes a fascinating study done in 2008, by Dr. Cassandra Godman. Her team took skin biopsies and extracted the normal tissue, then they examined the cells that lined the tiniest blood vessels in the tissue. Next, they put these cells in a petri dish and put them in a hyperbaric chamber and gave him a single hyperbaric treatment. Afterwards, they did a mass gene array analysis for 48 hours. What happened next was ground-breaking in better understanding the role that HBOT has on inflammation.
At the end of 24 hours 8,101 of our 19,000 protein coding genes in our 23 chromosomes were either significantly turned on or turned off & suppressed. That single HBOT treatment turned on the anti-inflammatory genes and the growth and repair hormone genes, and the largest cluster suppressed were the pro-inflammatory genes.
Our inflammatory reaction is inescapable when an injury occurs. But, when the patient receives hyperbaric oxygen therapy treatment after an injury (the sooner the better), it can have an overwhelming positive influence on the inflammatory reaction at any place along the spectrum. Harch continues to explain that the anti-inflammatory effects of hyperbaric oxygen are wide-ranging.
Harch spoke of the Navy’s experience with Hyperbaric Oxygen Therapy, as the Navy really kind of dominated this field for many years. The Navy reported that if you can get someone in a chamber within one hour of coming out of the water – when they’re symptomatic for decompression sickness – then the first hyperbaric treatment is curative in 90% of cases. Everyone thought that HBOT was treating bubbles in the brain at that point, but the reality is they were treating inflammation. We now know that it’s the inflammatory reaction in the brain after the bubbles passed that Hyperbaric Oxygen Therapy is treating; not the bubbles.
HBOT isn’t new, it dates back well over a hundred years. It’s still misunderstood, because many believe that HBOT therapy is a treatment for diseases. But, the reality is that HBOT is a treatment for disease processes, and it’s these processes that cause the diseases. We treat the inflammation that causes the disease processes, and in turn we treat the disease. That’s the power of Hyperbaric Oxygen Therapy.
