In this Healthed lecture, Prof Robert Mclachlan, Physician-Scientist; Director of Clinical Research, Hudson Institute of Medical Research; Principal Research Fellow, NHMRC; Deputy Director, Endocrinology Unit, Monash Medical Centre, explains that male hypogonadism, caused by intrinsic pathology of the hypothalamic–pituitary–testicular (HPT) axis, is an under-diagnosed condition not to be missed. By contrast, late onset hypogonadism (LOH), due to functional suppression of the HPT axis from age-related comorbidities, may be less common than previously believed. Prof McLachlan will provide an update on current evidence, appropriate testing, which patients should be managed with Testosterone supplementation and which by lifestyle modification alone. An overview of current Testosterone delivery options – cream, gel, patch or injection and which option suits which patient will also be covered.

Allowing men to express their emotional pain and marshalling support, offering helplines, support groups all help. Pay attention to longitudinal history when differentiating depression from bipolar disorder, and if you suspect this, avoid prescribing SNRIs and tricyclics. When the patient cannot see the pain they will cause when they die by suicide, we may have to take active measures even against the patient's will.

The pathway for suspected OSA in a patient is not simply to do a sleep study and prescribe a CPAP

Sleepiness is a complex issue and we need to explore the clinical context and other possibilities before arriving at the decision to perform a sleep study

GPs need to know how to use and interpret the results of a home sleep study and will probably need the input of a sleep physician to translate the results into a meaningful management plan

A Level 2 12-channel study is most informative and Medicare rebatable if the patient fits the criteria. However, GPs need to be able to interpret the results to ensure they are valid

GPs must give all patients with abnormal LFTs a positive diagnosis

Use ALT levels of 19 for women and 30 for men and investigate all who are higher

Triage at the GP level consists of calculators stratifying cirrhosis risk (links attached) where negative or very low numbers are of greatest value

Those with moderate to higher risks should have degree of fibrosis assessed with a fibroscan

A fibroscan result > 8.5 should alert us to refer to a hepatologist or liver clinic, equates to moderate fibrosis

It is a progressive disease. Rates of remission and recovery after a single episode are good (~80%), but rates decline with each subsequent episode.
